Overview
Inspection templates define the questions your inspectors answer during an inspection. Outcomes (also called compliance outcomes) are the results your inspectors pick when they close an inspection (for example, Pass, Fail, Follow-up). Percent calculators are an optional scoring tool that tallies points based on question answers.
This article walks through creating and editing templates, configuring outcomes, and setting up percent calculators.
Creating an inspection template
Click the Settings gear icon in the top-right corner of the page.
Click Inspection templates in the settings sub-sidebar.
Click Create Template (or the add/create button).
Fill in the template fields:
Name -- the template's display name.
Description -- optional context for inspectors.
Subject type -- what the template inspects: establishment, GCD, or vehicle.
Add questions (see a breakdown on field types below).
Click Save.
Adding and ordering questions
Questions are the body of the template. Each question has a field type, a label, and optional settings.
In the template form, click the blue + (plus) icon to add a new question.
The new question is inserted directly below the question where you clicked the plus. This lets you insert new questions between existing ones -- not just at the end.
Fill in the question's label and choose a field type (see below).
To remove a question, click the gray minus icon on that question.
Question field types
FOG inspection questions support these field types:
Short text -- one-line text answer.
Long text -- multi-line text answer.
Multiple choice -- single answer from a list of options.
Checkboxes -- multiple answers from a list of options.
Dropdown -- single answer from a dropdown list.
Date -- date picker.
Number -- numeric input.
Signature -- full signature capture. Requires the signatures feature to be enabled.
Simple signature -- lighter-weight signature capture.
File -- file upload (photos, PDFs, etc.).
Fixed text -- display-only text. Useful for section headers or instructions inside a template.
Conditional logic (show/hide questions)
Conditional logic lets a question appear or disappear based on the answer to an earlier question. This keeps templates focused: inspectors only see questions that apply to what they're finding.
How to set it up
Open the question you want to show or hide conditionally.
Expand the conditional logic section on that question.
Choose the action: Show (only display when condition is met) or Hide (hide when condition is met).
Select a reference question from the list. Only earlier questions with predefined answers appear here: multiple choice, checkboxes, and dropdown.
Choose the trigger answer -- the value that activates the show/hide.
Save the template.
When conditional logic is enabled on a question, the Required toggle turns off automatically. A hidden question can't be required.
Editing a template
Go to Settings → Inspection templates.
Click the name of the template you want to edit.
Update the template fields, add or remove questions, or change conditional logic.
Click Save.
Changes apply to new inspections only. Already-completed inspections keep the questions and answers they had when they were closed.
Archiving and restoring a template
Archiving hides a template from the inspection form's template dropdown without deleting it. Use it for templates you no longer want inspectors to pick but want to keep for historical reasons.
Archiving
Open the template from Settings → Inspection templates.
Click Discard (the archive button, shown when the template is saved).
Confirm the action. The template is hidden from active lists.
Restoring
In the Inspection templates list, toggle the view to include archived templates (or use the filter).
Open the archived template.
Click Restore. The template moves back to the active list and becomes selectable again.
Managing compliance outcomes
Outcomes are the values inspectors pick when closing an inspection. Each outcome has a name and a color so status shows up clearly in lists and reports.
Where to configure outcomes
Go to Settings → Inspections → Compliance
Review the existing outcomes in the list.
Adding an outcome
Click Create Compliance (or the add button).
Enter the outcome name (for example, Pass, Fail, Follow-up Required).
Pick a color. The color appears as a status indicator on inspection records and lists.
Click Save.
Editing an outcome
Click the outcome's name in the list.
Update the name or color.
Click Save.
Deleting an outcome
Click the outcome's name.
Click Delete and confirm.
Outcomes that are already assigned to inspections can't be deleted without reassigning those inspections first.
Setting up Percent Calculator Templates
Percent calculators are an optional scoring tool. Each calculator tallies components and produces a result the inspector sees inside the inspection. They're most useful for templates where the outcome depends on a weighted score.
How to set one up
Go to Settings → Percent calculator templates.
Fill in the total and components:
Total -- the label for the overall tally (for example, "Total points").
Components -- the scored sections. Each component has a name and an optional "calculate" flag.
Result -- the label for the calculated output.
Click the plus icon to add more components, or the minus icon to remove one.
Click Save.
Linking to an inspection template
Open the inspection template.
Turn on Percent calculator present (adds the calculator to the inspection form).
If every inspector must complete the calculator, also turn on Percent calculator required.
Click Save.
FAQ
Q: Can I reorder questions after they're added?
A: While there’s not a direct way to move questions around, you can delete and re-add them in the right position using the plus icon.
Q: Can an inspector change the template on an existing inspection?
A: No. The template is locked once the inspection is created. Delete the draft and start a new inspection with the right template.
Q: Do archived templates keep their past inspections?
A: Yes. Archiving only hides the template from the creation dropdown. All historical inspections remain intact and viewable.
Q: What happens if I delete an outcome that's in use?
A: You'll be blocked. Reassign the affected inspections to a different outcome first, then delete.
