Submitting a Survey

How to submit a backflow hazard survey in SwiftComply from the tester portal.

Overview

When your water purveyor has authorized you to perform backflow hazard surveys, you submit the survey directly in SwiftComply from the Surveys section. This article walks you through the full flow — from finding the location, to uploading your survey document, to submitting it for the authority's review.


Before You Begin

  • You must be logged in as a service provider user for the authority whose territory the location is in.

  • Your tester credentials (certifications, any required calibrations) should be up to date — though surveys typically don't have the same certification requirements as test reports, some authorities require a survey certification.

  • Have your completed survey form ready to upload (typically a PDF or image file).


Step 1: Start a New Survey

  1. Click Surveys in the left navigation.

  2. Click New Survey in the top right.


Step 2: Find the Location

  1. Use the search fields (Location Name, Location Address) to find the property.

  2. Click the search button.

  3. Select the correct location from the results list.

  4. If your authority allows tester-created locations and the one you need isn't in the list, click New Location and fill in the fields (Name, Address, City, State, Zip Code) to create it.

  5. Click Continue to Surveyor Selection.


Step 3: Confirm the Surveyor

  1. The surveyor list shows you (and any co-surveyors at your SP company if applicable).

  2. Click your name to select yourself as the surveyor.

  3. Click the continue button to proceed.


Step 4: Upload Your Survey Document

  1. Attach your survey file (the completed form, typically PDF, JPG, or PNG).

  2. Select a form field to associate the file with from the dropdown (if prompted).

  3. Click Upload to attach the document.

  4. Repeat for additional documents — different authorities may require photos, site sketches, etc.

  5. When all required documents are attached, click Next.


Step 5: Enter the Survey Date and Result

  1. Enter the survey date in YYYY-MM-DD format.

  2. Check the Pass checkbox if the survey passed (or leave it unchecked to record a failed survey, depending on your authority's conventions).

  3. Click Save & Submit.

  4. Confirm by clicking Yes in the confirmation dialog.


What Happens After You Submit

  • The survey's status changes to Submitted.

  • It appears in the authority's Surveys To Review queue on their Notifications tab.

  • The authority reviews your submission and either accepts or rejects it.

  • If accepted, the location's Survey Compliance and Survey Expiration update based on your authority's rules.

  • If rejected, you'll be notified with a reason. Open the survey, correct the issue, and resubmit.


FAQ

Q: How do I know if my authority supports tester-submitted surveys?

A: If your account has a Surveys section in the left navigation and a New Survey button, you can submit surveys for that authority. Some authorities restrict surveys to internal staff only — in that case you won't see these controls.

Q: I can't find the location I need to survey. What do I do?

A: First, try searching by address instead of name (or vice versa). If the location genuinely isn't in the system, check whether your authority allows you to create new locations. If the New Location button is visible, use it; if not, contact the authority to add the location before you can proceed.

Q: Can I save a survey mid-entry and finish later?

A: Depending on your authority's configuration, the flow may save your in-progress survey automatically. You can return to the Surveys list to find any saved (unsubmitted) surveys. Check the status column — anything in In Progress is still editable.

Q: I submitted the wrong file. Can I fix it?

A: Once a survey is submitted, it's read-only until the authority reviews it. If you realize you submitted the wrong document, contact the authority so they can reject it — you can then correct and resubmit.
